Healthcare for the Homeless

You deserve to be treated with respect and dignity, whether or not you have a place to live.

Homelessness is common and has devastating health consequences. CHA’s Healthcare for the Homeless meets you where you’re at to provide medical care and mental health support. We offer a low threshold approach to medical care and provide patient-centered, trauma-informed care with the goal of creating an acceptable and accessible medical home to all.

Our health care team of physicians, nurse practitioners, social workers and community health workers provide care in the community.  You will find us at shelter-based clinics, engagement and drop-in centers, and on the streets of Cambridge and Somerville. Our team works with human service agencies in Cambridge and Somerville to make sure you’re getting the support you deserve.

Services

  • Primary and episodic care
  • Walk-in clinics -- Cambridge Salvation Army,  Baycove CASPAR Emergency Service Center (for people also suffering from the disease of addiction) Somerville Homeless Coalition, and St. Patrick's Women's Shelter
  • Street Team outreach several times per week
  • Housing Case Conferences at Cambridge Multiservice Center
  • Dental screenings twice monthly at the Salvation Army site
  • Same day treatment at the CHA Dental Clinic – CHA Primary Care Windsor Street, Cambridge
  • Inpatient rounds at CHA Cambridge Hospital for patients experiencing homelessness; includes supporting discharge planning and treatment plans
  • Street Medicine Program -- provides a physician and nurse practitioner to work with the Baycove CASPAR First Step Outreach van, 3-4 sessions/week
  • Referrals to all area resources for other help and assistance
