Community Health – the faces of CHA

The essence of CHA is community. Our purpose is to improve everyone’s health and that begins with relationships with people like you who live and work in the communities where we provide care. CHA’s health services are integrated into the community through primary care, specialty care, programs and partnerships.

Community Health at its Best

Our community health efforts are integral to improving health through the lens of justice and health equity. They…

  • Provide clinical care in your neighborhood
  • Work with the community to understand overall health status
  • Build knowledge in the community through collecting and sharing data
  • Serve as the community wheelhouse, connecting and convening resources
  • Take action by initiating programs based on identified health needs and community engagement
  • Complete academic research with physicians, the community and interns
  • Create an environment where individuals can support their community health needs
