Our National Accreditation
In 2022, CHA received three-year accreditation in Complex Care Management (CCM) from the National Committee for Quality Assurance (NCQA).
CCM is a key component of CHA’s health care model - offering assessment, planning, coordination, evaluation and advocacy for patients (and their families) who have intricate social and behavioral needs.

“Congratulations to CHA's care management team for another excellent NCQA survey,” said Eleni Carr, CHA's Chief Care Integration Officer. “NCQA accreditation is the gold standard that validates our work - showing that CHA offers comprehensive care management services that meet or exceed rigorous national standards.
These allow us to maintain a consistently effective process to achieve better health outcomes, high patient satisfaction, and improved overall healthcare value."
