Somerville Health Foundation Seeks Grant Proposals to Serve Vulnerable Populations

SOMERVILLE, Mass — The Somerville Health Foundation, administered by Cambridge Health Alliance, serves to promote and support educational, research and outreach programs that improve primary care and preventive health services for residents of Somerville. The Foundation is soliciting proposals from 501(c)3 organizations for projects that will support the provision of primary care and preventive health services accessible to the residents of Somerville.

The current priorities for funding support are programs or projects with an emphasis on root causes of disease, health equity, health disparities, systemic racism and serving vulnerable populations across all ages among Somerville residents. Projects that address preventative health priorities will receive extra consideration.

Applicants must be a nonprofit organization or have a nonprofit fiscal sponsor. Collaborations are strongly encouraged. Proposals are due electronically by August 16, 2022, at 12 p.m. Visit https://www.challiance.org/community-health/health-improvement-team for an overview, applicant guidelines, and contact information.
