Hospitals and public health agencies across the U.S. carry out comprehensive assessments every few years to guide plans for improving community health. The Health Improvement Team collaborates with community members and partner organizations and participates in coalitions to examine and report on health needs and strengths in our communities. Specific community health topics, like food systems and oral health, are explored in deep-dive assessments as well. Click the button below each cover image to access these reports.
The Oral Health Assessment provides a novel approach to community oral health needs assessment in Everett and Malden. It was led by the CHA Department of Dental Medicine and Oral Health, with collaboration from the Health Improvement Team.
Food systems are an important part of community health. The Malden Community Food Assessment is a collaboration of Bread of Life, CHA, YWCA/Malden is Moving! and the Metropolitan Area Planning Council.
The North Suffolk Public Health Collaborative is an initiative of the communities of Chelsea, Revere, and Winthrop. CHA participates as a member of the steering committee.
Assessment begins with listening. Our approach to assessment revolves around asking questions that matter, lifting up community members’ priorities, centering equity, and collaborating to create solutions. We developed a framework to guide our approach, informed by experiences, lessons, and practices from our own communities and others around the country. Our framework is always evolving.
