Referral Information for Providers

Cambridge Health Alliance has top medical specialists and surgeons who work at our three hospital campuses - in Cambridge, Somerville, and Everett - and outpatient clinic sites throughout our service area.

Outpatient Medical Referrals:

1. Schedule an appointment directly with the specialty office.
2. Then fax the patient's referral form and authorization number to the Alliance's Central Referral Office located at 10 Beacon Street in Somerville, MA. Please send the authorization prior to the appointment date.
