Medical records let us keep track of your medical history and care
In the past, medical records were written on paper and kept in folders. Today, we use a state-of-the-art electronic medical record system. This is a safe and more efficient way for us to keep track of your health needs and give you better care.
Do you need your medical records?
You can get copies of your records by printing our Medical Records Release Form. Please fax this to the number listed on the form or bring in person. For more information, please call our Medical Records department at the following numbers:

There may be a cost for copies
To access your records, you must sign a request form that you can get from our HIM/Medical Records Department (see above). If you want copies, we will charge a reasonable fee for them. We will also charge you for our postage costs, if you want us to mail the copies to you.
